About İstanbul Hand and Microsurgery Clinic

Our group has been serving Hand Surgery as a team work for many years. Our philosophy is performing hand surgery with preferred methods at the same level in other advanced centers of the world, and supporting it with academical activities and scientific publications.

Our team consists of both Orthopaedic and Plastic Surgery based Hand Surgeons in order to deal with all problems of upper extremity from shoulder to fingertip.

Carpal Tunnel Surgery (1 hour)

ervus medianus, one of the two main nerves responsible of the hand/fingers movements and sensation travels along with nine tendons underneath a bridge called ‘’transvers carpal ligament’’. This bridge and bones on the bottom create a tunnel called ‘’carpal tunnel’’. For any reason if median nerve represents nerve symptoms such as numbness and/or motor inabilities this is called ‘’carpal tunnel syndrome’’.

Although the reason for nerve compression cannot be found in most cases, hormonal changes, pregnancy, rheumatoid diseases, masses in the tunnel etc.. could ease to show up the symptoms.

First symptoms can be felt mostly at nights. Numbness in thumb, index, long and (half of)ring fingers, clumsiness, pain(in the late stages) are main complains of the patients. In late cases, the muscle bulk between palm and thumb base lost its prominence. In time, the symptoms expand to whole day.

The syndrome is generally diagnosed by physical examination and electrophysiological studies(electromyography - EMG). In cases which are doubtful about their etiology(the reason of the disease) additional radiological methods are required to reveal previous fractures or possible masses.

In some cases wrist splints may lessen the symptoms. Steroid injections may decrease swellings around the nerve and help for diminishing the symptoms.

Surgery is the treatment of choice for patients whose symptoms last more than 2-3 months despite conservative methods. The roof of carpal tunnel is opened to relieve compression to the median nerve. Some severe cases need neurolysis (releasing nerve sheet under microscope)as well. Endoscopic carpal tunnel surgery (closed method)may be applied in selected cases. Patients should know that all symptoms may not be eliminated in some severe and delayed cases.

Dupuytren's Contracture Treatment (1 hour)

here is a layer of tissue,called fascia, under the skin of palm which helps to keep the skin from sliding around when we grip things. In some people, this tissue thickens and shrinks, and pulls on the skin and and on the fingers(

figure 1
).

Dupuytren’s disease is observed usually in men over 40. Frequency is more in diabetic ones. Ring and small fingers are affected more. Any relationship between patient’s occupation and the disease is not proven.

Swollen and bent fingers are seen in the examination. Extension is limited depending on the severity of the disease. Palmar noduls are typically painless. Sometimes it is confused with trigger finger at the early stages. Patients have serious difficulties in hand washing, hand shaking, wearing gloves etc…..  Progress rate changes from patient to patient.

Existence of nodules is not an indication for surgery itself. Steroid injections on nodules may decrease the rate of progression in selected cases. If the angle between the affected finger and palm is equal or more than 70 degrees, surgery is considred. The affected part of the palm fascia and its extensions are removed. To be able to protect vital structures such as vessels, nerves and tendons is important. Sometimes skin grafts from other parts of the body may be needed.

Wound care, massage, exercises and splinting is important to get the best result and prevent recurrence.

Biography:
İsmail Bülent Özçelik was born in Konya in 1972. He went to Istanbul University School of Medicine between 1989-1995. He did his residency at Clinic of Orthopedia and Traumatology in Sisli Etfal Training and Research Hospital between 1996 and 2000.
In 1996, Dr. Ozcelik took a Basic Microsurgery Course at the Sisli Etfal Training and Research Hospital . During his residency , he also work at Hand Surgery Department of Aksaray Vatan Hosipital. In 2003, he jointed to IST-EL Hand Surgery, Microsurgery a Rehabilitation Group, and started to Gaziosmanpaşa Hospital in 2005. In 2007, he received education an wrist artroscopy at German Clinic für Handchirurgie Bad Neustadt an der Saale for a while.
Dr. Ozcelik has worked for Turkish Society for Reconstructive Microsurgery as a council member for six year. He was also in the organization commitee of Europan(EFSM) and Asian Pasific (APFSRM) mikrosurgery congresses in 2016 and 2018. He took part in organization and instruction of Basic Mikrosurgery Courses of the national society during his executive membership. Currenthy , he is assigned an instructor in IRCAD Strasbourg wrist artroscopy courses. He has been in the organization commitee of EWAS arthroscopy courses which was held in Istanbul. Dr Ozcelik has been found as the chairman and speaker in many congresses and courses. While he received his associate professorship in December 2012, he received his professorship title in January 2018.

Dr. Berkan MERSA was born in Karsiyaka/Izmir in 1967. He finished Ege University Medical School in 1991. Dr. Mersa did his residency at Plastic and Reconstructive Surgery Departmant of İstanbul University Medical School between 1992.
Dr. Mersa did a research and clinical fellowsh about Reconstructive Microsurgery and Peripheral Nerve Surgery in Contern Vingihia Medical School (Norfolk, VA, USA). Dr. Mersa worked with world-renowned microsurgeon Dr Julia K. Terzis at this clinic between 1998-1999. Since 1999, Dr. Mersa has been performing hand surgery and microsurgery.
