AI - Artificial Insemination

Insemination is a method of infertility treatment where semen is injected into the uterus during the release of the egg. Insemination is often combined with a mild hormonal therapy, to mature 1-2- to eggs in the ovaries.Insemination can be done if the fallopian tubes are open and the semen is of good quality. In order to determine the ability of sperm to fertilize an egg is recommended to perform a sperm function test. Before the insemination, the semen is washed to selectively collect the most motile and normal structured spermatozoids. The use of donor sperm is also possible.Despite the mild hormonal medication it is possible that several follicles are matured, and the treatment must be terminated due to the risk of multiple pregnancy or it can be converted into IVF treatment.Hormonal therapy always involves a certain small risk of hyperstimulation, i.e. ovarian overreaction, therefore it is very important to regularly examine ovaries with ultrasound.

Blastocyst Transfer

Extended culture of embryos to the blastocyst stage is a method used in fertilization treatment where more information is available about the vitality of the embryos. Blastocyst culture can be done both in IVF and ICSI treatments. In usual treatments, the embryos are cultured for 2-3 days after the fertilization. The embryos can be divided into 4-8 cells before the transfer to the uterus. At this stage, the selection of the most vital embryos from all embryos of good quality may be difficult.By culturing embryos for a longer period of time (the prolonged cultivation), we are able to choose the most vital ones for transfer, i.e. embryos that have continued to divide and develop. In extended culture, the embryos are cultured for 4-5 days prior to the transfer to the uterus. At this stage, the embryo already has more than one hundred cells, and it is at either the morula or blastocyst stage at the moment transfer. This corresponds to the development stage of an embryo formed in natural conception as it passes through to the uterus. At the morula stage the embryo forms a cluster of cells where we no longer can count the exact number of cells. At the blastocyst stage the first differentiation of cells has occurred. The inner cells in the thickened area of the blastocyst develop into fetal tissue, and the surrounding cells, known as trophoblast, develop into the placenta at the fetal end. At the blastocyst stage the embryo is ready to attach onto the lining of the uterus once it has hatched out of its membrane. The likelihood of implantation of an embryo thus developed to this stage is great. This enables us to transfer a single embryo only and thus decrease the risk of a multiple pregnancy. The blastocyst culture of embryos is a good option in cases where the implantation of embryos has not occurred during earlier treatments. The disadvantage of extended culture is that only 40-60% of embryos reach the blastocyst stage, and it is possible that all of the embryos cease t

Egg Donor

A woman who does not have any contraindication for pregnancy or delivery can opt for fertilization treatment with donated eggs. The results of fertilization treatment with donated eggs are extremely good. Donated eggs enable the conception and the birth of a healthy child in many families when this is no longer possible with the woman’s own eggs. We choose such donor for recipient whose physical features, such as the colour of hair and eyes, body build match those of the recipient. The egg donation program of the AVA clinics do not share the eggs, the recipient is given all the eggs that her chosen donor produces during one treatment cycle. The number of eggs varies depending on the donor’s reaction to the stimulation. The donated eggs are fertilized with the partner’s sperm, either fresh or frozen. We screen our egg donors carefully. They are healthy women aged 18-34, who voluntarily offer their help to grant the chance of motherhood to anonymous recipients. Most of our donors are mothers themselves and wish to share that experience with those who desire it. In many cases the donors have witnessed the grief caused by infertility among family and friends. In the process of choosing donors, the psychologists of our clinic meet and discuss with the possible donors. All our donors have been tested for any abnormalities in their chromosomes, HIV, contagious hepatitis as well as the most common sexually transmitted diseases and also have been examined by narcologist. The treatment with donated eggs is an option to conceive if a woman has failed to get pregnant with her own eggs. Fertilization with donated eggs is a big step for couples to take, and to come to terms with the idea often requires time and professional help. Therefore we recommend all of our recipient couples to discuss the issue with either our specialists at the clinic.Treatment using donor ovum provides opportunity to become pregnant if it has proved not possible by of own ovum of patient. Egg Freezing

As a result of IVF and ICSI treatments, there are often more embryos obtained than can be transferred into the uterus at a time. The rest of the embryos of good quality can be frozen and stored in liquid nitrogen (kryo-preservation) for a long period of time. The freezing of embryos enables subsequent attempts at pregnancy without a new hormonal stimulation. Before the transfer of the frozen embryos the endometrium of the uterus is increased by oral medication. The frozen embryos are thawed the day before the planned embryo transfer. Not all embryos survive the freezing and thawing process and it is possible that embryo transfer will not take place. The survival rate of embryos frozen at our clinic is 60-70%. It is also possible to freeze sperm for subsequent use. In AVA CLINIC also actively using vitrification as a new method of embryos and genetic material freezing. Vitrification - a special, extra-fast method of oocytes and embryos cryopreservation, which almost completely excludes the formation of ice crystals and minimize the damage of cells. So far as the embryo consists of water for 95%, during slow freezing microscopic crystals of ice are formed in cells, which can injure the cells of embryos, and due to its high speed of freezing water immediately turns into a gelatinous state, which excludes the possibility of damage to most cells.

Fertility Specialist Consultation

ExaminationsFirst consultationAt your first appointment with our doctor together you will go through all factors affecting conception as well as all your prior examinations or treatments. Our doctor will explain all the possible treatment options and the likelihood of conception after various treatments. The needs of each client are individually evaluated during the consultation session. If any further investigations are needed, a tailored schedule is planned to best meet the client’s expectations. Examinations prior to treatment.Examinations prior to the treatment aim at explaining the reasons why a natural conception does not occur. It is important to find the factors behind infertility, and the results determine the course of treatment. It is advisable to bring any previous results to the consultation session since they are all taken into account and new examinations will be carried out if necessary. The investigations need not to take a long time and the treatment can begin quite soon if the client so wishes.Female testsA doctor will plan an individually tailored schedule of examination depending on received data. The development and release of a female egg from the ovaries, determining whether the fallopian tubes are open and the structure of the uterus are important factors that need to be examined. Ultrasound is used to detect any abnormalities in the uterus, and this can often be done at the first appointment.Sometimes we need to do hysteroscopy, the visualization of the uterine cavity, and only very rarely laparoscopy, the visualization of the abdominal cavity. The maturing of eggs can also be detected with ultrasound, which is timed according to the estimated day of egg release during the menstrual cycle. The so-called SSG ultrasound can help determine whether the fallopian tubes are open.A small amount of liquid is injected though the cervix and the passage of the fluid into the uterus and through the fallopian tubes is monitored. The sperm fertilization capability is evaluated according to the analysis. The quality of semen is a significant factor when determining the choice of an appropriate method of infertility treatment. It is necessary to book an appointment for the semen analysis at the clinic since it is important that the sample is analysed within an hour after its collection.Private facilities are available for the male partner to provide a semen sample at our clinic. The sample may also be produced at home into a sterile container provided by the clinic but it must arrive in the laboratory for an analysis within half an hour. ICSI - Intracytoplasmic Sperm Injection

Intra Cytoplasmic Sperm Injection, (ICSI)As in vitro fertilization, inracytoplasmia sperm injection microfertilization is an efficient method of infertility treatment. This method is more frequently used for treating male infertility. It is used if the number of spermatozoids, motility, morphology or results of sperm function test are clearly abnormal, resulting in an impaired ability of semen to fertilize. In this case fertilization can be ensured with micro fertilization, also known as the ICSI method. Each mature egg is injected with one single spermatozoid. With the help of the ICSI method, fertilization can be ensured in almost all cases, including cases where the conventional IVF method has proved unsuccessful.Micro fertilization is also the only available option to use clients’ own sperm in case if no sperm can be found in the semen sample, it can be extracted directly from the testes as a result of TESA manipulation. However, not all mature eggs get fertilized even when ICSI is used. The success rate of egg fertilization of both IVF and ICSI methods is approximately 70 - 80%

IVF - In Vitro Fertilisation

There are two methods of fertilization, depending on the semen quality and results of sperm function test, either by means traditional IVF method, or with a help of micro-fertilization (ICSI). Fertilization in vitro (IVF) is the type of infertility treatment when ovum is fertilized out of female body.Currently fertilization in vitro is the most efficient type of infertility treatment. If the semen is normal or close to normal in regard to the number of sperm, its motility and morphology, the conventional IVF method can be used for egg fertilization. The processed semen sample is placed in the dish in which the eggs are cultured. The sperm swim to the eggs and into a special dish attach onto their surface fertilizing them. But for fertilization there should be only one spermatozoid.

Ovulation Induction

Ovulation means the release of an egg from the ovaries in the middle of the menstrual cycle. Some women in their reproductive age might ovulate infrequently or not at all. Ovulation induction is a treatment which facilitates the release of the eggs from the ovaries, with the help of hormonal therapy. A very mild hormone medication is used to help produce 1-2 mature eggs in the ovaries. The growth of the follicles is monitored with ultrasound. Once the follicles have matured enough, the ovulation can be scheduled with a single hCG-hormone injection. The moment of ovulation injection can be timed with intercourse or insemination. This treatment requires that the ovarian tubes are open and the semen is of good quality. Donor sperm can also be used in insemination.Despite the mild hormone medication, it is possible that several follicles are produced, and the treatment must be terminated due to the risk of multiple pregnancies, or it can be converted into IVF-treatment. Hormonal therapy always involves a small risk of hyper stimulation, i.e. ovarian overstimulation, which explains the importance of ultrasound scans during the hormonal therapy.
